Voshell's Pharmacy 3700 Edmondson Ave. (not marked but this was in Baltimore Maryland), in excellent++ unused new old stock condition. Has some spotting along the right edge. Size is 4 1/8" x 5 1/8". Scans show the front and back. The black border is so all the edges can be seen and is not part of the receipt itself. All of the items I sell are guaranteed to be 100% authentic and original.
