This note is very heavily circulated and has the following: very heavy folding and wear (also has many heavier creases and many areas of wrinkled folding); overall staining and darker areas of spot/line/section stains (also has many smudges, many dark and dirty areas, grease and rust stains, areas of discoloration, foreign substance including writing in reddish ink or crayon); heavy edge and corner wear (including corners rounded and/or split/frayed, edges rough in many areas); many edge indents/nicks (several tears exist and some are larger); very heavy surface wear and fraying (areas of paper loss exist); no holes that I could see.
